In criminal appeals, ineffective assistance of counsel is when your lawyer's conduct falls below the standard of legal professionalism. If your lawyer messes up and it important to the case or your rights, then your conviction will be reversed. But if your lawyer just made the wrong strategic decision instead of making a mistake that no reasonable lawyer would make, or if the mistake didn't really affect anything in the trial, then the appellate courts will not reverse the conviction.
